The My Community and Home Care Program at Tallangatta Health Service is funded to provide a range of services designed to support you, or the person you care for, to stay active and independent and living at home for as long as possible.

It aims to help people stay active, involved and doing as much for themselves as possible is proven to help people stay more independent with a greater sense of wellbeing.

Services provided through this program include:

• Domestic assistance
• Home maintenance
• Home modifications
• Personal care
• Meal preparation
• Individual support to take you shopping, attend activities, outings and attend appointments
• Respite care
• District nursing
• Meals on Wheels
• Social group activities

Referral is via My Aged Care portal, GP or specialist referral. This service also accepts self-referral via 02 6071 5238 or THSMCHCAdministration@ths.org.au
